I authorize Red Willow Production Company to change my address of record. This authorization will remain in effect until Red Willow Production Company receives a change request in the web-based system (and has had a reasonable opportunity to act on it), or until I receive written notice from Red Willow Production Company that the web-based system for address changes has been discontinued.

I authorize Red Willow Production Company to deposit my accounts payable/revenue payments directly into the account I have linked. I also authorize the financial institution holding the account to post all such deposits. This authorization will remain in effect until Red Willow Production Company receives a change request in the web-based system (and has had a reasonable opportunity to act on it), or until I receive written notice from Red Willow Production Company that the web-based system for banking changes has been discontinued.
